Summary:Sofia presents to session reporting that she is not doing “so good” and specifies to the provider that she is experiencing a lot of “sad days” and days that she is “not ok”. The provider proceeds by asking Sofia a series of questions to informally assess for symptoms of major depression. Sofia endorses numerous symptoms of major depressive disorder, including feeling chronically sad and frequently crying, diminished interest/pleasure in activities she usually enjoys, loss of appetite, hypersomnia (i.e., sleeping 10+ hours a night), diminished energy/fatigue, and isolation. Although Sofia reports that she is still capable of taking her kids to school and making them food daily, she admits she does not do “much else”. Sofia’s symptoms are clearly causing her significant impairment in multiple areas of her life.
