Summary: | In assessing for depression, Cooper describes his current mood as unhappy and angry. At first, Cooper ascribes these feelings to persistent pain in his back and stomach. Lately, he is easily irritated and has low energy after sleeping only 3 or 4 hours at night. He finds it hard to concentrate or perform simple tasks at work, resulting in bills left unpaid and doctor visits postponed. Just sitting down with the clinician today was an enormous effort for Cooper. Cooper does not believe that doctors can help, as they never have in the past, nor did they help his mother who attempted suicide in the past. Overall, Cooper feels stuck, has poor self-esteem, and does not believe the future can get better. |
