Summary:Jennifer is visibly tense and restless during the interview and has difficulty concentrating. Lately, she has found most day-to-day activities to be challenging and overwhelming to the point where they have impacted her functioning. This excessive worrying has been going on for at least the last nine months, possibly starting when she had to leave her job. Jennifer has not had any changes in her relationships or underlying health. Jennifer also describes difficulty sleeping, restlessness, and racing thoughts. Because of these symptoms, Jennifer has difficulty concentrating or finishing simple tasks such as making tea because of the compounding anxiety it creates. When asked about any history of trauma, Jennifer describes growing up with her brother who was an addict and his leaving the family. The discomfort she demonstrates during the interview occurs throughout the day for Jennifer, affecting her appetite and enjoyment in visiting friends or going out. In assessing for psychotic features, we learn that Jennifer does not hear or see things that aren’t there. However, Jennifer does describe a compulsion of touching wood to prevent bad things from happening.
